Family Service Case Manager- Adult Service Coordinator

Department: The Harry & Jeanette Weinberg Building
Location: Washington, DC

SOME (So Others Might Eat) is seeking a full-time Family Service Case Manager to work with Adults in a residential setting. The ideal candidate will enjoy planning life skills events, working with adults 18-24 to help them succeed and motivating them to be there best

Position Description: Provides case management and support services for adults and children living in Family Housing. Develops and coordinates programming for adults on life skills and personal and professional development.

Qualifications: Must have a Bachelorís Degree; 2-3 yearsí experience in social services; case management experience and program development experience.

Required License/Certification: CPR/First-Aid, CPR for Infants, Driverís License and access to vehicle.

Schedule:Full-time, Monday-Thursday 11:30 am to 8:00 pm and Friday 9:00 am to 5:30 pm.

Knowledge, Skills, and Abilities:

  • Knowledge of principles, methods, and procedures for motivational enhancement interviewing and for career counseling and guidance.
  • Knowledge of family/group/individual behavior and dynamics and sensitive to diverse cultural backgrounds
  • Ability to write effective SMART goal plans and document progress
  • Ability to educate and teach residents how to access and utilize community resources
  • Lead and facilitate workshops and life skills training
  • Effectively organize and prioritize work tasks with effective time management skills
  • Understanding of issues of trauma and mental health
  • Establish and adhere to strong professional boundaries
  • Ability to professionally document progress notes and maintain resident files.

Expected Contributions:

  • Case Management: Using strength-based perspective and motivational enhancement theory, collaborate with families and individuals to develop goals, monitor, and report progress. In regular intervals conduct meetings, school, and home visits when needed. Meet with education support staff and outside service providers. Perform intakes and annual assessments to incorporate findings into case management services. Maintain progress notes and program charts using appropriate documentation standards.
  • Program Activity Development: Develop and coordinate activities for adults on life skills and personal and professional development. Promote, market and ensure attendance for all program activities and initiatives. Identify resources and establish partnerships to assist in activities. Evaluate program effectiveness and document resident feedback. Develop and submit quarterly community events schedule/calendar and family activity plans.
