Care Coordinator (Chronic Care)

SOME (So Others Might Eat) exists to help homeless and poor individuals and families of Washington, DC. Our more than 30 programs throughout the District provide comprehensive services that lead to mental and physical health, continued sobriety, job training, living-wage employment, and affordable housing. We meet immediate needs with food, clothing, and healthcare and offer the tools one needs to live with hope, dignity, and greater independence.

SOME (So Others Might Eat) is currently seeking a full-time Care Coordinator (Chronic Care) for our Population Health department.

Position Overview: The Care Coordinator, Chronic Care provides comprehensive care coordination for high-need Medicare members with co-morbid chronic physical and behavioral health conditions. Participates in assessment and care plan development working within a team approach to improve health outcomes, to reduce avoidable inpatient and emergency department encounters, to lower cost, and increase engagement in and satisfaction with a total experience of care.

Qualifications

Required: High School Diploma and 2+ years of experience in community-based health or mental health setting

Preferred: Bachelor's Degree and 3+ years of community-based work; experience with homeless adults, co-occurring psychiatric and substance use disorders, Medicaid populations desired.

Schedule: Monday - Friday, 8:00 am - 4:30 pm

Required License/Certification: First Aid/CPR, valid driver's license to be able to drive clients.

Knowledge, Skills, and Abilities:

  • Knowledge of mental health, addictions, and common chronic physical health conditions.
  • Ability to travel throughout the District of Columbia to perform outreach and linkage, including but not limited to homeless service settings, individuals' homes, public spaces frequented by homeless individuals, hospitals, and emergency rooms.
  • Knowledge of individual behavior and dynamics, societal trends and influences, ethnicity, cultures, and diverse backgrounds.
  • Ability to be sensitive to diverse backgrounds and set aside personal opinions and implicit biases. This includes their basic principles, values, ethics, ways of thinking, customs, practices, and their impact on human culture.
  • Demonstrated excellent judgment in providing clinical support and care.
  • Ability to communicate effectively in writing and verbally as appropriate for the needs of the audience. Talking to others to convey information effectively.
  • Ability to educate and teach clients how to access and utilize community resources.
  • Ability to evaluate the level of care and implement service accordingly.
  • Ability to learn and comprehend clinical interventions and training acquired for appropriate implementation.
  • Ability to effectively document clients' progress towards goals electronically.
  • Knowledge of principles, methods, and procedures for motivational enhancement interviewing. Being aware of others' reactions and understanding why they react as they do.
  • Ability to learn and utilize Motivational Enhancement Practice.
  • Draft clinical notes for review.
