Senior Case Manager

Compensation:
We offer our employees a competitive compensation and benefits package that reflects our organizational culture, mission, and core values. The hourly range for this position is $59,000 to $62,000k and is commensurate with experience.

SOME (So Others Might Eat) is currently seeking a full-time Family Services Adult Case Manager - Adult Service Coordinator within our Family Services department located at Weinberg House in NE Washington, DC.

Position Description: The Weinberg House Senior Case Manager is responsible for overseeing the case management and programmatic functions of the site, in conjunction with managing a case load. The position provides supervision of the Family Services Case Managers and the Program Assistant. This position is also responsible for staff development and evaluation, overseeing case management plans, and developing programs for adults and youth that are creative, engaging, educational, and that promote family stabilization. Partners and works collaboratively with community resources for housing, employment, substance abuse treatment, mental health treatment, training, and makes referrals as appropriate.

Qualifications:

Required: Bachelor's Degree in related field; 2+ years of case management experience; supervisory experience. Experience working in a residential setting is preferred.

Schedule: Mon, Wed, Fri 8:00am-4:30pm and Tues & Thursday 11:00-7:30

Expected Contributions:

  • Program: Responsible for oversight and support of the case management program at Weinberg, ensuring of program effectiveness and client outcomes are met. Responsible for providing case management. Ensure necessary follow-up, referrals, and documentation are completed in a timely manner. Ensure strength-based practice is fostered within the program and modeled during case management sessions.
  • Provides oversight of Program Assistants and youth services program. Ensuring structure and a safe environment for the children. Provides guidance on activities, curriculum, educational and social development initiatives.
  • Responsible for program implementation of resident programs and activities. Ensuring quality programming that fosters community spirit, stability, life skills and over all well being.
  • Responsible for tracking, reporting and communicating program effectiveness by documenting clear and reliable program reports, outcomes success stories and statistics.
  • Works collaboratively with Program Manager to meet the administrative needs of the program as needed as well as perform Program Manager responsibilities in their absence. In addition, to working collaboratively with the Clinical Service Coordinator to ensure resident mental health well being.
  • Contribute to strategy development and strive to improve department functions and performance; Assist in development of standards and goals as appropriate
  • Submit administrative paperwork to Human Resources, Finance, Compliance, and CQI; Refer to SOME Employee Handbook or policies for guidance
  • Maintain professional documentation and ensure the program is in compliance with SOME CQI standards
  • Ensure compliance with all funding streams as it relates to case management program
  • Schedule staff work hours, as needed, and monitor for time and attendance; Approve time cards and requests for leave
  • Provide direction and guidance to staff members on protocol and procedures
  • Define work requirements, assign tasks to staff members, and review work for quality and accuracy
  • Provide regular feedback on performance, monitor individual performance, conduct performance evaluations, and take steps for performance improvement as needed
  • Provide coaching and training for staff, assist staff with work processes and policies, and encourage opportunities for professional development.

Reports to: Program Manager

Position Designation: This position is designated as Safety Sensitive. You may be subject to drug testing prior to or during your employment with SOME. In this position, you may be disqualified from employment based on the presence of marijuana in test results, even if you possess a medical card authorizing the use of medical marijuana.

Physical Requirements: Must be able to lift up to 20 pounds. Requires looking at a computer screen for several hours a day. May be required to sit for long periods.
