Program Assistant, Population Health

SOME (So Others Might Eat) provides material aid and comfort to our vulnerable neighbors in the District of Columbia, helping them to break the cycle of poverty and homelessness through programs and services that save lives, improve lives, and help to transform the lives of individuals and families, their communities, and the systems and structures that affect them. We meet immediate needs with food, clothing, and healthcare and offer the tools one needs to live with hope, dignity, and greater independence.

The Program Assistant provides vital administrative support to department programs by ensuring timely and accurate billing compliance and federal program adherence. Verifies participants insurance and demographic regularly for eligibility, input billing entries to EMR, support coordinators with CRISP, HMIS and other area databases.

This position is located at our main campus on O Street in NW Washington DC.

Covid-19 Considerations: In alignment with DC Health, COVID-19 vaccinations, and boosters are mandated for personnel working in SOME’s healthcare-designated programs, which currently include Medical, Dental, Population Health, Behavioral Health Services (BHS), Exodus House, Jordan House, Kirwan House, Kuehner House, Leland Place, and Mary Claire House. Schedule: Monday through Friday 10:00 am - 2:00 pm.

Qualifications

Required: 3+ yrs. administrative experience providing support for multiple programs simultaneously in an environment that serves at risk populations (i.e., homeless, domestic violence, disabled); experience providing services in community or mental health setting.

Required License/Certification: CPR/First-Aid

Expected Contributions:

  • Maintain timely and accurate daily notes in the electronic health record.
  • Verify participant insurance and support recertification biweekly
  • Create, maintain and enter information into database
  • Identify, track, and accurately report program statistics, especially related to structural barrier mitigation (housing, employment, etc.)
  • Assist with year end reporting data
  • Assist with client appointments i.e. scheduling, bus route assistance, medical transportation coordination. Document concerns and follow-up on missed appointments.
  • Develop clinical notes on progress of client treatment in Electronic Medical Records.
  • Coordinate communication amongst team members, outside agencies, and physicians regarding client care

Knowledge, Skills & Abilities Required:

  • Knowledge of mental health, addictions, and common chronic physical health conditions
  • Ability to communicate effectively in writing and verbally as appropriate for the needs of the audience. Talking to others to convey information effectively.
  • Knowledge of electronic health records usage and management
  • Proficient with MS Office, including Word, Excel, and Outlook
  • Ability to learn and comprehend clinical interventions and training acquired for appropriate implementation
  • Ability to effectively document generic notes electronically

Reports to: Program Director, Population Health

Position Designation: This position is designated as Safety Sensitive. You may be subject to drug testing prior to or during your employment with SOME. In this position, you may be disqualified from employment based on the presence of marijuana in test results, even if you possess a medical card authorizing the use of medical marijuana.

Physical Demands: Must be able to lift up to 20 pounds. Requires looking at a computer screen for several hours a day. May be required to sit for long periods. Must be able to travel to events and meetings offsite.
